Step 1: Review the Terms and Conditions for Kuali Ready subscribers and the contract between Kuali and the service provider (Exhibit B).

Step 2: Complete the form below and click "Submit".

Step 3: Once your instance has been created, the functional owner you specify below will receive an email with login instructions to Kuali Ready.

Subscribing Institution
Contract Representative: Please enter the contact information for your institution's representative for subscription matters.
Subscription Type: Please select a subscription type below. If your institution is part of a system which has already subscribed to Kuali Ready, then please select "sponsored member" and provide the name of that organization in the field below, otherwise leave it blank.
Functional Contact: The functional contact will be the first user created in Kuali Ready as system administrator and the only individual authorized to submit support tickets. Generally, this individual should be the primary person responsible for continuity planning at your institution.
